In the context of the Safety Matters App, safety risks found during ad hoc inspections are known as ‘issues’. Documenting an issue using the app only requires a few steps. Start by logging in to the Safety Matters App, and you should automatically be directed to the ‘Dashboard’ page.

Select the blue circle icon with a ‘+’ symbol inside of it and select the option to ‘Report an Issue’.

You will now be asked to select the facility location at which the issue has been found.

Now you are on the ‘New Issue’ page, and you will be asked to select which safety issue you would like to document.

Fill out a short explanation of the problem in the ‘Issue description’ text box, and take a photo or attach a photo you have already taken.

Press ‘Submit’ to confirm your submission.

Now you may choose whether to view the issue you have created, document another issue, or return to the main menu.
